What is a Health referral?

A Health Referral is a scheme set up to encourage those people who would most benefit from activity in terms of health to access suitable and beneficial physical activity often offered at a reduced rate and tailored specifically to their own needs.

If you have any of the following medical conditions and want to improve your health, a health referral may be for you:-

• Anxiety/Depression

• Osteoporosis

• Elderly Clients

• Overweight

• Arthritis

• Asthma

• Cystic Fibrosis / Chronic Obstructive Pulmonary Disorder

• Multiple Sclerosis

• Chronic Fatigue Syndrome / Myalgic Encephalomyelitis

• Thyroid

• Diabetes

• Angina

• Myocardial Infarction

• Hypertension

• Back Pain

• Joint Replacement

• Pregnancy

• Varicose Veins

• Pulmonary Vascular Disease

• Parkinson’s disease

• Eating Disorders

• Cancer

• Downs Syndrome

• Cerebral Palsy

• Spinal Cord Injury

• Stroke

To obtain a form you must see your health professional and then contact New Life Health and Fitness Great Cornard Sports Centre Head Lane Sudbury Suffolk CO10 0JU Tel:- (01787) 374861.  We will then make you an appointment with our highly Qualified Exercise Referral staff, who will arrange a bespoke workout to take into consideration all your medical needs.

All it costs is the price of a prescription of £7.10 and the concession rate every time you visit the gym £3.00.
